LifeStreamUK is committed to ensuring that your privacy is protected, any information given to us by which you can be identified by will only be used by us in accordance with this privacy statement. This Privacy Policy forms part of your Terms and Conditions and your contract with us for the booking and arrangement of your appointment.

LifeStreamUK is committed to protecting your safety and privacy, and takes its responsibilities regarding the security of customer information very seriously. This privacy policy explains what personal data we collect about you, how we use it, who we disclose it to, and how we protect your privacy.

Collecting and using your information

Information about you is collected when you engage with LifeStreamUK by booking an appointment, contacting us via telephone or contacting us by email. We use your personal data to inform you of any changes or let you know any useful information prior to your appointments.

In order to provide you with a high standard of care we need to hold personal information about you. This personal data comprises:

  • Personal details such as your full name, date of birth, address, email address, telephone number and some details about your pregnancy. We sometimes have to share your data with your NHS care provider to make sure that in the event of us identifying any problems, you receive the correct care.
  • Notes of conversations/incidents that might occur for which a record needs to be kept
  • Records of your booking (Customer Booking Form, T&C’s)
  • We have a commitment to collect, process, store and share your data safely and securely. This will not be shared with any third parties other than the NHS and/or local authorities and this will be simply to care and protect you.

Your right under GDPR

You as an individual have rights as follows:

The right to access your personal data and information that we hold. This will allow you to be aware of and verify the lawfulness of the processing

  • The right to be informed about processing of your personal data
  • The right to have your personal data corrected in case of inaccuracy and to have incomplete personal information completed. You can make a request for rectification verbally at our studio or in writing to us at managers@lifestreamuk.co.uk. We will respond to a request within a maximum 28 days. In certain circumstances we may refuse a request for rectification for example if we reasonably believe that the request is excessive or unfounded.

  • The right to removeyour personal data. This is also known as the ‘right to be forgotten’. The right is not absolute and only applies in certain circumstances.

Requesting access to your personal data

You have a right to request access to the personal data that we hold about you. This could include information relating to appointments you have had with us.

If you would like to request a copy of your personal data, please email us to managers@lifestreamuk.co.uk stating your full name, details of your appointment and which data you require access to. We will always make sure that it is you that we are in contact with before we allow any data to be released.

We are committed to taking appropriate measures to protect your personal data.

The information that you provide to us will be held within the premises of LifeStreamUK.

We will retain your personal data for as long as necessary in accordance with UK law, this is purely for medical purposes only in the event of any medical concerns at birth.
